Service Description:

Access to health care support at home or in the community in a culturally sensitive manner
- intake and elgibility
- client assessment
- case management
- referrals to Algonquins of Pikwakanagan Home Care
- referrals to Ontario Health atHome for home nursing, * occupational therapy, * physiotherapy
- provides patient advovcay as requested by clients for appointment, etc
- maintains an equipment lending system for medical or equipment supplies on a short term basis for home and community care and home care clients

Referrals can be made in person, by phone or in writing by client, caregivers or other health providers

Must be a residing, registered status Algonquin of Pikwàkanagàn First Nation member * no age restriction * people living with a disability * chronic or acute illnesses * convalescing at home individuals * the elderly
