Service Description: Supports for people living with prediabetes or diabetes

Education Clinics: certified diabetes educators, including registered dieticians and nurses * work in cooperation with physicians, pharmacists and family members * individual and group appointments * referrals and telehealth sessions with specialists for advanced care

Education on:
  • diabetes control
  • healthy living and eating
  • hyperglycemia (high blood sugar)
  • hypoglycemia (low blood sugar)
  • common emotions
  • physical activity
  • medications

Outreach Sites: screening assessments to determine risk of developing diabetes * individual and group education and counselling

Renfrew County Pediatric Diabetes Education Program: at Pembroke Regional Hospital or through Telehealth * in partnership with the Children’s Hospital of Eastern Ontario (CHEO)

Fees: Free
Application: Self referral or physician/healthcare provider * call and leave a message to register, response within two business days
Eligibility / Target Population: 19 and over * 18 and under for pediatrics
