Service Description:

Support for people living with prediabetes, Type 1 and Type 2 diabetes * Individual (in-person and virtual) consultations on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * registered nurse and dietitian on staff * Diabetes education program provides individuals with the tools they need to manage and treat their diabetes

- Individual counselling with the nurse and dietitian
- Virtual group education about pre-diabetes and diabetes
- Insulin initiation and adjustments (with a physician’s referral)
- Non-insulin injectable medication teaching and initiation
- Support for insulin pump therapy
- Special interest topic workshops
