Service Description:

Supports for people living with diabetes (Type 1, Type 2, Gestational) * Patients learn to manage their conditions through self management techniques * Clinicians educate patients about how to reduce the likelihood of diabetes-related complications: medication management, nutritional goal setting, exercise recommendations
Patients are guided to manage intensive insulin therapy plans and the latest technology in managing their diabetes
