Service Description:
|
The program provides comprehensive support for individuals living with Type 1, Type 2, and Gestational Diabetes
Patients are supported in developing the knowledge and skills needed to effectively self-manage their condition * Clinicians provide education and guidance focused on reducing the risk of diabetes-related complications, including medication management, nutrition goal setting, and physical activity recommendations
Patients are also supported in managing intensive insulin therapy and are introduced to current diabetes technologies that assist with monitoring and managing their condition

No cost to participate with a valid Ontario Health Insurance Plan (OHIP)
|

Application:
|
A physician referral is required for consultation with a diabetes specialist * may self-refer to see a Diabetes Educator or to attend an information session * All visits by appointment only * bring valid health card to appointment

Adults with diabetes * Children with diabetes in collaboration with CHEO
