Service Description: Integrated community mental health support services for individuals with a severe and persistent mental illness and/or concurrent disorder and those who are homeless or at risk homelessness with complex needs

Services include: Intensive Case Management, Housing Support, Court Diversion/Support, Vocational Support, Health Promotion, Hoarding Support * Services are offered in the client's home and in the community

Community Homes for Opportunity (CHO): a recovery-oriented person-centred supportive housing program for people with serious mental illness who may require 24 hours supportive care * The program is intended to assist tenants by providing appropriate housing and support services to achieve and maintain housing stability that is safe and affordable

Resource Centres provide day programming for adults with a serious mental illness and/or concurrent disorders as well as those socially isolated

Mental Health Promotion provides workshops on a variety of mental health and well-being topics

Services and Programs:

Fees: Training workshops fees * None for client services * Some charges may apply for groups as well as for Resource Centre activities/outings
Application: Referrals to can be made by self, family, hospital staff, physician, psychiatrist, mental health, and other social service agency * Call to be directed to the appropriate administrative office * complete the referral from online at Caredove Forms or if assistance is needed call 613-933-5845 and ask for information and referral for your area * Mental Health Promotion also provided * See website for further information on available workshops, campaigns and events
Eligibility / Target Population: Adults and youth 16 years and older, with a serious mental illness and/or concurrent disorders for the Intensive Case Management Services and Resource Centres

No restrictions for Mental Health Promotion Workshops
