Service Description:

Services and supports provided to youth and their family made available in their own community * each youth and family have an individualized plan * the plans are based on the strengths of the child/family, community members and professionals to support them in dealing with complex problems

Application:

A professional considering the referral should speak with the family and describe the process to determine if they would like to try it voluntarily * the referring worker should then contact the Youth Services Bureau's Intensive Family Support coordinator to make a referral

Eligibility / Target Population:
|
Youth 12-17 years old and their families who have complex needs and may be considered high risk in any two of the following life domains: housing, family, social, behavioural/emotional/psychological, educational/vocational, safety, legal, health as well as crisis intervention, spiritual, cultural or financial
