Service Description: Bilingual individual/family centered service for adults with a developmental disability and children with a developmental disability or autism spectrum disorder

The programs include:

Case Management: home and community-based support services to children who have a developmental disability and/or autism spectrum disorder and their families, and to adults diagnosed with a developmental disability.

Residential and Community Services (RCS): case management for youth in extended care with the Children's Aid Society (previously known as Crown Ward) receiving residential services with private supports * Resource Coordinator helps to monitor quality of services received and transition to adult services

Developmental Services Ontario Eastern Region (DSOER): access point for adults with developmental disability * aid with application, understanding services, access to funding, information and referral to appropriate community services

Resource Room: information and referral to community resources * help completing service application forms * registering for programs or services

Eligibility / Target Population: For children: Documentation confirming a diagnosis is required * Documentation such as school or psychological assessment or medical reports will be required stating the above information in order to determine eligibility
