Service Description:
|
Outreach services * assesses the needs of homeless people in Ottawa
- Transportation: Emergency based transportation service for homeless and shelter-staying clients such as transport from street to shelter, shelter to more appropriate shelter, and to and from hospitals where an ambulance is not needed and a nurse or social worker can confirm the client has been seen and discharged by a doctor.
- Supplies (for clients who live on the street and choose not to come in a shelter only): Life-sustaining necessities such as a sleeping bag, boots, jackets, socks, gloves, water, etc. * does not provide food
- Referrals/Assessment : Can meet the client at their location and provide an assessment for advocacy or referral to the social services network of Ottawa

To report the location of a homeless person who may need help, call the City of Ottawa and the intake worker will contact the outreach van * for general information, call the Ottawa Booth Centre

Eligibility / Target Population:
|
Homeless individuals 16 years and older
* Transportation: There must be a demonstrated need for the service such as the client cannot bus, is too intoxicated to travel safely or has no other means to get there * Not for transportation to and from private residences and the client must be mobile enough to get in and out of the vehicle on their own
