Service Description:
|
Program for seniors who require interdisciplinary professional assessment and treatment to promote safe optimal function and independence in the community * short-term intervention to maximize function
Assistance with concerns including: falling * getting up out of a chair * understanding * medications * preparing meals * dressing and bathing * safety at home * living arrangements * mood * memory * loneliness, isolation * maintaining social contact

Fees for transportation to/from the program * Service is Covered by the Ontario Health Card

Application:
|
Referral from family doctors, members of the Geriatric Assessment Outreach Team, or referral through an in-patient admission to the Queensway Carleton Hospital.

Eligibility / Target Population:
|
Seniors, 65 years of age or older, who require assessment and interventions from at least two of the following disciplines: occupational therapy, nursing, physiotherapy, social work,etc. * The program is suitable for patients who are independently mobile with or without a mobility aide (cane, walker, wheelchair) and those who require minimal physical assistance. Persons who require constant supervision or may demonstrate inappropriate social behaviour cannot be managed in the program
