Community paramedicine program * in-home visits from paramedics * some treatments are provided in the patient's home * health assessments (diabetes, heart problems, chronic obstructive pulmonary disease, vital signs, etc.) * environmental assessment * urine and blood tests * coordination of care and referrals to community resources * Palliative care assistance * IV antibiotic in home assistance

Call to book an appointment * if the telephone is not picked up, leave a message and the call will either be returned within a few hours if during business hours, or the next morning if the call was placed outside of business hours * referrals can also be sent by email or by fax
