Service Description: Hospital-based outpatient day program with a focus on rehabilitation
  • therapy by regulated health professionals (nurse, respiratory therapist, pharmacist, dietitian, physiotherapist, social worker)
  • exercise program
  • group education sessions (optional) held weekly
  • referrals to other services

Fees: Covered by OHIP
Application: Cardiovascular: Referral from family doctor or health care provider Pulmonary: referral from hospital respirologist
Eligibility / Target Population: Adults 18 years or older with a cardiovascular disease, a pulmonary disease, or who have had a cardiac event, procedure or surgery
