Service Description:
|
Access to health care support at home
- respite
- personal care such as bathing, grooming, dressing and exercise programs
- meal planning and preparation
- companionship, motivational activities and socialization
- household management such as laundry and light housekeeping
|

Fees:
|
$12 per hour (minimum 2 hour visit) up to 15 hours per week
|
|
|
Application:

Self or health professional referral * agency and client will determine the schedule based on availability
