Service Description:

A free, confidential, and voluntary home visiting program for pregnant women and families with children from birth until up to transition to school * team of Public Health Nurses, social workers and Home Visitors provide support and information on topics such as having a healthy pregnancy * caring for a new baby * infant feeding and nutrition * growth and development * parenting concerns * referrals to other community services

Referrals to the program can be made by phone by physicians, other professionals and by families themselves * Healthy Babies Healthy Children Program Referral Form is also available at https://www.rcdhu.com/healthy-living/child-health and can be faxed * A Public Health Nurse will contact the family to explain the program and complete the screening process
