Service Description: Fulfills research, education and advocacy roles around severe and persistent mental illness and homelessness Multidisciplinary on-site mental health screening * assessment and intervention * including crisis intervention for the clients of partner agencies * ongoing education and support to the staff of partner agencies * linkage of clients of partner agencies to appropriate mental health * health, and addictions services * hospital based emergency and acute care services

Application: Referral from partner agencies
Eligibility / Target Population: Adults 16 years and older who are homeless or at risk of homelessness * have a severe and persistent mental illness including a concurrent disorder of addictions and mental illness * use the services of partner agencies including emergency shelters, rooming houses, residential care facilities, drop-in centres, and community health centres * are not supported by another mental health service
