Service Description: Team of mental health professionals who provide specialized services to individuals with severe and persistent mental illness * provides support and accompaniment in the person's living environment

Eligibility / Target Population: Persons 18 years and older with chronic and/or persistent mental illness * also, persons who have frequent or prolonged admissions to hospitals for mental health reasons

Persons who have a significant and apparent functional disorder that is defined as: the inability to keep up with daily activities, to ensure personal safety and to participate in existing services
