Service Description: Support and rehabilitation to patients and their families during treatments and follow-up. The team includes: social workers, dietitians, physiotherapists, speech and language pathologists, psychologists and psychiatrists

Groups: Coping with Cancer Stress * Connections 18-35 * Caregiver Support Group

Education sessions: Nutrition during Cancer Treatments * Breathe * Lymphedema Education Class



Fees: Set fees, covered by the Ontario Health Card
Application: Referral by an Oncologist or Health Practictioner * self-referral also accepted
Eligibility / Target Population: Patients 18 years and older of The Ottawa Hospital Cancer Centre and their families
