Registered Nurses (RN) with extensive knowledge and experience assist patients and families as their Care Manager for both acute situations and chronic conditions
Holistic approach may include:- community support, nursing, palliative care
- accompaniment to medical appointments
- oversight of treatments and procedures
- communication with physicians and other providers
- assistance with access to community resources
- research diagnoses and treatment options
- investigation of alternatives based on patient's needs
- specialty in Parkinson's Disease management, including EMBRACE PD Clinic
- Palliative Care and support with End-of-Life care needs
