Primary Health Care Desktop Resource Guides |
The primary health care desktop resource guides were developed by a working committee with representation from the Champlain Local Health Integration Network, primary care physicians and other health care professionals from across the Champlain region.
These web-based desktop reference tools are for clinicians and other primary care health service providers. The template for the desktops has been developed and includes a number of key headings which are standardized for each clinical desktop and will also include a short list of related services, phone/fax and web addresses.
The intent of the desktops is not to present an exhaustive list of resources, but to provide information regarding the type and range of services that exist and the community services which could be consulted to assist in providing information and referrals to meet the client needs.
For each desktop the resource list is on one side and a number of helpful clinical/screening tools are on the other side. The desktop is formatted to fit standard size paper.
The web and pdf version of the clinical desktop will be updated regularly.
