Supports for Adults living with prediabetes or diabetes
Education Clinics provide:
*certified diabetes educators , registered dieticians and nurses who work in cooperation with physicians, pharmacists and family members
- individual and group appointments
*referrals and monthly telehealth sessions with specialists for advanced care
Education on:
- diabetes self-management
- healthy living and eating
- hyperglycemia (high blood sugar)
- hypoglycemia (low blood sugar)
- common emotions
- physical activity
- medications
